A 55-year-old woman developed painful, red facial and neck lesions with fever two days after starting inhaled indacaterol/glycopyrronium for COPD. Blood tests showed neutrophilia; biopsy confirmed Sweet syndrome.

Lesions improved with corticosteroids after stopping the drug. This is the first reported case linked to inhaled therapy, highlighting the need for primary care awareness of rare drug reactions and the importance of ruling out systemic disease.
